What Twist Inserts Do
Every Pos-T-Vac twist-lock cylinder has a standard opening at the base. For most men the standard opening creates a workable seal — but for men with more pubic tissue, anatomical variation, or anyone seeking maximum vacuum efficiency, the standard opening can allow non-penile tissue (scrotal skin or supra-pubic fat) to be drawn into the cylinder during vacuum creation. This reduces suction consistency and can cause discomfort.
A bushing insert narrows the cylinder opening to fit more precisely around the shaft of the penis at the base. The tighter fit:
- Improves vacuum seal — less air escapes around the base during pumping
- Prevents or reduces scrotal or pubic tissue draw-in
- Increases comfort against the body during therapy
- Improves vacuum consistency — the motor or pump works more efficiently
Inserts are included with new BOS-2 and BOS-3 kits. This listing is for replacement inserts or for men who need a different size than what came with their system.

Sizing
Four sizes are available — from largest (widest opening) to smallest (narrowest opening). Most men start with one of the middle sizes. If you experience significant tissue draw-in with no insert, start with size 2 or 3. If you've used an insert before and want a tighter fit, go one size smaller. Do I need a bushing insert to use my Pos-T-Vac?
Not necessarily — new systems include inserts, and many men use the cylinder without one. If you're experiencing inconsistent vacuum, discomfort at the base, or tissue draw-in during therapy, trying an insert is the recommended first adjustment before replacing the cylinder.
My BOS-2 / BOS-3 came with inserts — why would I need to order more?
